The only items in this code that you need to change are what is in RED. It's pretty simple but if you have problems please email me. Or you can avoid problems of my pic host site going down and copy the below pic and placing it on your own server. If you choose to do this URL where your copy of the pic is stored goes in the purple spot in this code.
